In order that such an impression may be of real value it is essential that the case be approached in a mental mood which is altogether free from bias. The ordinary academic method of taking notes, as you know, is that in which the history of the patient comes first. The history of the patient is often taken without any special regard to the local condition of the disease as it presents itself to the observer the history having been taken, the examination of the diseased part follows. If you consider the details of such a method, you will see at once that the examination of the actual disease is not approached with quite a free mind, because you will have been biassed, more or less, by what the patient has told you with regard to the history of the complaint. Now, if too much importance is attached to the history in certain conditions of disease, you are very liable to be misled in the matter of diagnosis, because, as I have just said, of the bias which you are apt to get from the account which the patient gives.
