Patient safety is fundamental to the provision of health care in all settings. However, avoidable adverse events, errors and risks associated with health care remain major challenges for patient safety globally. The Seventy-second World Health Assembly in 2019 adopted resolution WHA72.6 on global action on patient safety and mandated for development of a global patient safety action plan. This global action plan was adopted by Seventy-Fourth World Health Assembly in 2021 with a vision of “a world in which no one is harmed in health care, and every patient receives safe and respectful care, every time, everywhere”. The purpose of the action plan is to provide strategic direction for all stakeholders for eliminating avoidable harm in health care and improving patient safety in different practice domains through policy actions on safety and quality of health services, as well as for implementation of recommendations at the point of care. The action plan provides a framework for countries to develop their respective national action plans on patient safety, as well to align existing strategic instruments for improving patient safety in all clinical and health-related programmes.

The Department of Health estimates that one in ten patients admitted to NHS hospitals will be unintentionally harmed (a rate similar to other developed countries), due to incidents such as an injury from a fall, medication errors, equipment related incidents, record documentation errors and hospital acquired infections. About half of such incidents could have been avoided, if lessons from previous incidents had been learned. This NAO report examines the progress being made in the NHS to improve the patient safety culture, to encourage incident reporting and to learn lessons for the future. The report finds that most trusts have developed a predominantly open and fair reporting culture at the local level, driven largely by the Department of Health's clinical governance initiative and more effective risk management systems. However, a 'blame culture' still exists in some trusts, and there have been delays in establishing an effective national reporting system. There is scope for improving strategies for sharing good practice and for monitoring that lessons are learned.

There is an urgent need to develop and improve low cost technologies for wastewater treatment. Simultaneously treating wastewater and producing duckweed in a pond system is, therefore, an attractive solution contributing to both environmental protection and food production. Duckweed has excellent qualities: a high protein content, a high growth rate and is an easy crop to handle. The small plant turns nitrogen from wastewater into a food source. This thesis reports on the effect of different operational variables, like anaerobic pre-treatment, the combination of algae and duckweed ponds and pond depth. Improved nitrogen removal was obtained through the combination of duckweed ponds with algae ponds. Duckweed pond systems could be designed with shallow depth without affecting nitrogen removal efficiency. This research is the result of the cooperative effort between the EIDENAR, Univalle, Cali, Colombia and the UNESCO-IHE Institute for Water Education in Delft, the Netherlands.
